HBT: Santana's struggled since Mets no-hitter

Did the 134 pitches take a toll? After getting lit up by the Yankees last week, Johan Santana gave up four runs in five-plus innings Thursday versus the Rays in his second start since his no-hitter.

The 134 pitches Santana threw in shutting down the Cardinals were a career high for the left-hander, who was handled carefully during the first two months in his first season back from shoulder surgery. Pitching on six days? rest last Friday, he gave up six runs and four homers in five innings in a loss to the Yankees. Today, working on five days? rest, he got a win, but he allowed four runs in five-plus innings.

It actually should have been worse. Santana loaded the bases with no outs in the sixth, giving up a single to Hideki Matsui, a double to Ben Zobrist and a walk to Matt Joyce. Jon Rauch took over and managed to strand all three runners by getting two strikeouts and then a groundout.

Santana threw just 51 of his 95 pitches for strikes in this one. He issued four walks, his second highest total of the season (he walked five in the no-hitter).

Santana?s ERA has jumped from 2.38 to 3.23 since the no-no and would be higher if not for Rauch?s effort. The Mets have to be hoping its an aberration, not a trend. Santana and R.A. Dickey have been an outstanding one-two punch so far, but Santana needs to return to May form to keep that up.

Spain borrowing cost crisis: Rate at euro-era high

(AP) ? Spain's benchmark borrowing rate hit its highest level Tuesday since the country adopted the euro currency, after ratings agency Fitch downgraded 18 banks on Tuesday and investors continued to find more questions than answers in the country's decision to seek help for its ailing bank sector by tapping a ?100 billion ($125 billion) eurozone bailout fund.

The yield on Spain's 10-year bond yield rose to hit 6.81 percent in afternoon trading, according to data provider FactSet, while stocks seesawed between positive and negative territory and ended the day almost unchanged, up 0.1 percent.

The bond rate seen as a measure of a nation's financial health fell back to 6.67 when markets closed. That's the same level as Spain's previous record ? set on May 30 as the country's economic woes multiplied, and last November after the then-ruling Socialist Party was ousted by the conservative Popular Party now struggling to keep the national afloat financially. This brings Spain's borrowing costs dangerously high to 7 percent ? close to the level at which Greece, Ireland and Portugal sought international bailouts.

Spain agreed last weekend to take a European bailout for its banks, tapping into a ?100 billion ($125 billion) euro area bailout fund, but investors are worried it will not solve the country's problem as the government may have trouble paying the money back.

Fitch said in a statement that its downgrade of the banks was a result of a previous downgrade of the Spanish sovereign debt on June 7. Fitch said it had conducted stress tests, both on the Spanish banking sector as a whole and on individual banks, updating results from tests done in 2011.

The ratings agency said the weakness of the Spanish economy would continue to have a negative effect on business volumes "which, together with low interest rates, will place pressure on revenues."

There has been growing concern that an increasingly large amount of Spanish government debt is being bought by its banks as the country finds fewer and fewer international buyers for its bonds. As Spain's banks continue to struggle, weighed down by their toxic property loans and assets, the government is finding it increasingly harder to sell its bonds.

One hope among eurozone politicians is that the ?100bn loan facility will help shore up Spanish banks' balance sheets, thereby giving them back the ability to loan out money to businesses and individuals ? and also buy more government debt. However, Spain is in danger of being trapped in a vicious debt circle. The ?100 billion loan facility will increase the Spanish government's debt load and it will have to find more buyers for its bonds ? which will send borrowing costs even higher. This could push Spain's government to ask for a bailout of its own.

The rescue package for Spain's crippled lenders was announced Saturday by finance ministers from the 17-country euro area, but the exact amount the country's banks will receive has not yet been published.

It is not yet clear where the euro area bailout loans will come from. If the money comes from the existing eurozone rescue fund, the European Financial Stability Facility, its repayments will have the same priority as the all the other private bond investors. However, if the funds are to come from the new bailout facility, the European Stability Mechanism, its bond repayments will be given a higher priority than everyone else's ? which could mean that other debt would be less likely to be paid off. That could make bondholders less willing to buy Spain's debt or demand a higher interest rate to compensate for the added risk of losses.

Spain will wait for the results of two independent audits of the country's banking industry before saying how much of the ?100 billion it will tap. The bailout loans will be paid into the Spanish government's Fund for Orderly Bank Restructuring (FROB), which would then use the money to strengthen the country's teetering banks.

In a report released late last week, the International Monetary Fund estimated Spain needs around ?40 billion to prop up banks hurting from an unprecedented real estate boom that went bust.

Investors also want to know whether Spain will ask for a safety margin of extra money to cushion itself against a further shock, such as a deterioration in the economy.

While Spain's bailout is designed to prop up its banks, investors are also worried that the Spanish government might eventually be forced into asking for a bailout to help it pay its way. Recession-hit Spain, which has the eurozone's fourth-largest economy with unemployment of nearly 25 percent, may be too big for the eurozone's rescue funds to handle.

Spain is also getting punished because of fears Greek elections on Sunday will hand a victory to the radical left-wing Syriza party, campaigning on a pledge to refuse to comply with terms of that country's bailout package. This could eventually push Greece out of the euro, further destabilizing the currency group.

"The idea of the Spanish bailout was to calm the market in case the Greek elections do not turn out as the EU would like," said Gary Jenkins, director of Swordfish Research Ltd.

"However the end result was to create more volatility and create more concern."

Once it became clear that the rescue money for Spain's banks would not solve that country's problems, investors have turned their attention to Italy, whose economy is larger than Spain's.

Italy didn't suffer through a real estate bust, so its banks are in better shape. But like nearly half of the countries in the euro, its economy is shrinking, making it difficult for the government to chip away at a mountain of debt. Italian bond yields also rose to worrying highs on Tuesday, hitting 6.02 percent, its highest level since January.

"Although Italian banks are relatively sound compared to the Spanish counterparts, without the heavy weight of toxic real estate bubble and are much less exposed to the government bonds, the real question is whether the country can grow itself out of the recession," said Anita Paluch of Gekko Global Markets.

3-center study identifies main causes of unprofessional behavior among hospitalists

Unprofessional behavior among hospitalists is rare, but those who do behave poorly share common features, according to research published in the Journal of Hospital Medicine.

American researchers spoke to 77 Illinois hospitalists - doctors who provide care tailored to the needs of hospitalized patients as a general internist, rather than focusing on an organ, disease or a specific patient age-group.

The three-center study found four key factors or patterns underlying unprofessional behavior: making fun of others, conduct in the learning environment (i.e. texting during educational conferences), workload management (i.e. celebrating transferring a patient) and time pressures (i.e. signing out early).

"The discrepancy between what is taught about professionalism in formal medical education and what is witnessed in hospitals has received increasing attention" says co-author Dr Vineet M Arora from the Department of Medicine at the University of Chicago, USA.

"As a result, regulatory agencies require training programs and medical schools to evaluate the learning environment and its impact on professionalism. Our research suggests that educational interventions could be tailored towards hospitalists with certain job types or personal characteristics."

Most of the hospitalists who took part in the study (79%) completed their residency after the year 2000. More than half (57%) were male and had worked with their current hospital group for one to four years (61%). They were asked to grade more than 30 unprofessional behaviours on a simple, five-point scale commonly used in research to measure attitudes or opinions. Hospitalists reported witnessing unprofessional behaviour more frequently than taking part in it.

The most common unprofessional behaviors that hospitalists reported participating in were: having non-medical or personal conversations in patient corridors (67%), ordering a routine test as urgent to expedite care (62%), signing out a patient over the phone when it could have been done in person (41%) and making fun of other physicians to colleagues (40%). But when the researchers looked at how many observed those same behaviours in colleagues, the figures were much higher, at 80%, 80.5%, 66% and 67.5% respectively.

The four key factors or patterns underlying unprofessional behavior accounted for 76% of the variations in the survey responses and had the greatest impact on the overall findings.

Job characteristics - such as clinical time, administrative time and night work - together with age and hospital site were all associated with different patterns of unprofessional behavior.

For example, hospitalists with less clinical time were more likely to report behaviors relating to making fun of others. Young hospitalists and those who had any administrative duties were more likely to participate in behaviors related to workload management. And hospitalists who worked nights were more likely to report behaviors related to time pressures.

"Interventions to promote professionalism should take institutional culture into account and should focus on behaviors with the highest participation rates" concludes Dr Arora.

Dr. Kevin O'Leary, a co-investigator from Northwestern University summarized "Although this study found that unprofessional behavior was thankfully rare, such behavior is unacceptable in a professional hospital setting and needs to be addressed."

OGDEN ? Trial has begun over whether to make Ogden?s highly controversial use of restraining orders against members of one street gang permanent.

Attorneys for some of the alleged gang members have fought it, claiming it violates their First Amendment right of free association. Lawyers for Ogden police and Weber County argue that it?s an effective way to cut down on crime.

?Graffiti?s down 50 percent in the city,? Weber County Attorney Dee Smith told FOX 13. ?Other crimes are down anywhere from 10 to 40 percent.?

The restraining orders have been served on hundreds of members of the gang, prohibiting them from associating with each other in public within Ogden city limits. It also limits their abilities to carry guns or graffiti tools.

?It stops them from seeing certain people in any places in any circumstances,? said Randy Richards, an attorney for some of the alleged gang members. ?It prevents them from going out after certain period of time. It prevents them from engaging in a number of different legal activities, normally legal ? but now illegal for them.?

On Monday, Second District Judge Ernie Jones heard testimony from police officers who have dealt with the gang. Inmates who are members of the gang were also brought up from the Utah State Prison to testify about their tattoos and identify themselves in photographs.

Lawyers for the alleged gang members tried to warn them that their words may come back to haunt them in future criminal prosecutions. It was revealed in court that federal prosecutors had been looking at the gang for a potential RICO (racketeering) case. However, nothing had come of it.

Brent Vigil smiled and laughed at some of the pictures, but when asked about what his tattoos said, he turned serious and replied: ?It?s just a gang-related tattoo.?

Elmer Maes Jr. was declared a ?hostile witness? for refusing to answer some questions in the civil trial.

?I got my injunction paper!? he barked at Smith while on the witness stand. ?I been served! What?s the point of this? If I don?t have a say, then I don?t want to say.?

FOX 13 was there when police served the restraining orders back in 2010. Police went door-to-door, tracking down people they said were documented gang members and served them with the papers.

None of the alleged gang members served with restraining orders showed up to court on Monday. Their lawyers claimed that if they did, they would be in violation of the injunction.

?It?s clearly unconstitutional,? argued attorney Michael Studebaker.

A woman named Mia, who refused to give her last name and asked that her face not be shown on TV, told FOX 13 her husband was served an injunction after doing prison time for gang crimes. She said the restraining orders go too far, preventing members of the same family from seeing each other ? even after they have paid their debt to society.

?We raise our family. We pay our house taxes. We own our vehicles. We own our home. We were born and raised here,? said Mia. ?What are we supposed to do? Sell our home and leave just because the gang unit feels like they want to harass people??

The trial resumes on Tuesday. The judge could decide the case by Thursday. The legal impact of the restraining orders is being watched closely by other police agencies in Utah.

Smith told FOX 13 he would not rule out serving other gangs in the Ogden area with similar injunctions, if this is upheld by the courts.

?Once this one?s complete, then we?ll make decisions after that about who?s next,? he said.

(AP) ? Apple says it's introducing a laptop with a super-high resolution "Retina" display, setting a new standard for screen sharpnessj.

The new MacBook Pro will have a 15-inch screen and four times the resolution of previous models, Apple executive Phil Schiller told developers at a conference in San Francisco.

Apple already uses "Retina" displays ? with individual pixels too small to be distinguished by the naked eye ? in its latest iPhones and iPads.

The new MacBook will cost $2199 and up, and is part of a general revamp of Apple's personal computer line announced Monday.
